1. Name of the national level organization applying for funding
2. Description of the organization's in-depth understanding and experience in providing technical assistance and support to state and territorial governors and their policy advisors
3. Explanation of how the organization plans to improve public health, health care, and health care delivery at the state and local level
4. Plan for building capacity to address public health issues and enforcing regulations to improve the public's health
5. Strategy for promoting health equity and preserving/improving public health
6. Proposal for facilitating communication and data/information-sharing between HRSA and state, territorial, and/or local officials
7. Approach for understanding state, territorial, and/or local needs, priorities, and perspectives
8. Plan for supporting capacity-building activities at the state, territorial, and/or local levels to strengthen the health care safety net and advance public health goals
9. Definition and explanation of the term "capacity-building" and how it will be implemented in the proposed project
10. Identification or development of solutions to public health challenges faced by state governors and leveraging HRSA investments
11. Strategy for improving access to quality health care for medically and economically vulnerable populations, with a focus on under and uninsured populations, maternal health disparities, people with unmet behavioral health needs, youth, justice involved populations, and other vulnerable groups
12. Outline of tailored technical assistance to be provided to state and territorial governors and their policy advisors
13. Plan for communication and information sharing between HRSA and state/territorial governors and their policy advisors
14. Proposal for training, data collection, sharing, and analysis, publication development, convenings, and other activities that will enable state and territorial governors and their policy advisors to operate effectively
15. Detailed implementation plans for policy initiatives that improve access to quality health care for highest-need populations intersecting with HRSA programs
16. Explanation of how the organization will implement technical assistance activities and tools to address public health challenges and share best practices
17. Demonstration of how the proposed project will have a nationwide impact, if feasible
18. Compliance with program requirements and expectations outlined in the grant description.
