1. Applicant's name and contact information

2. Institutional details (name, address, etc.)

3. Proposed research career development program plan

4. Description of how the program will prepare clinically trained vision scientists for independent research careers

5. Explanation of how the program will expand and strengthen the community of clinician investigators engaged in vision research

6. Description of the clinical trial(s) proposed by the scholar (lead investigator, ancillary trial, or research experience in another investigator's trial)

7. Confirmation that the proposed clinical trial is a NIH-defined clinical trial fulfilling the minimal risk trial requirement

8. Justification for the minimal risk categorization of the proposed clinical trial

9. Consultation details with NEI program staff regarding the human subjects aspect of the application

10. Recruitment plan to attract individuals from underrepresented racial and ethnic groups as well as individuals with disabilities, in accordance with NOT-OD-22-019 guidelines. This should include plans for prospective PIs/PDs, mentors, and scholars.

11. Budget outline for the proposed program

12. Timeline for the program implementation

13. Statement of commitment to complying with ethical guidelines and regulations

14. CVs or biosketches of key personnel involved in the program (PIs/PDs, mentors, scholars)

15. Letters of support or recommendation from relevant individuals or organizations.

CanExport SMEs

Amount of Funding

Up to 50% funding to a maximum $50,000 grant per project;
Applicants may receive up to 50% of eligible costs for up to $10,000 to $50,000 in grant funding per project;
The applicant is responsible for the remaining 50% of costs;
The annual CanExport contribution maximum is $99,999 (in the event of multiple projects approved in the same government fiscal year);
A group of related companies cannot receive >$200,000 in funding in a single fiscal year. Projects budgets should be between $20,000-100,000; and
Applicants can apply for funding multiple times as long as their prior CanExport application has been completed all reporting is finished.

CanExport SMEs

Eligible Applicants

To be eligible for the CanExport program, Canadian businesses must:

Maintain 1 to <500 full-time employees;
Earn annual revenues ranging from $100,000 to $100,000,000;
Be federally incorporated federally or classified as a limited liability partnership (LLP);
Create eligible project expenses of $20,000 or more; and
Be financially stable and able to pay all project expenses not covered by the program.
