To apply for the Rural Maternity and Obstetrics Management Strategies (RMOMS) program, applicants must provide the following information:

Proposed collaborative improvement and innovation network: Describe the network structure and membership, including the rural hospitals and communities involved in providing maternity and obstetrics care. Explain how the network will identify and implement evidence-based and sustainable delivery models for maternal and obstetrics care.
2. Approach to enhancing access to maternal and obstetric services: Outline strategies for improving access to preconception, prenatal, pregnancy, labor and delivery, and postpartum services in rural hospitals. Include plans for coordinating and sustaining the delivery and access of these services. Describe any proposed training programs for healthcare professionals who do not have specialty maternity care.
3. Collaboration with academic institutions: Explain how the network will collaborate with academic institutions to access regional clinical expertise, including specialty expertise and provider support through telehealth services. Discuss the role of academic institutions in identifying barriers to providing maternal health care and developing strategies to address these barriers.
4. Addressing disparities in infant and maternal health outcomes: Describe plans to assess and address disparities in infant and maternal health outcomes among rural racial and ethnic minority populations and underserved populations. Explain how the network will collect and analyze data to monitor progress in achieving this goal.
5. RMOMS Focus Areas: Select one or more of the RMOMS Focus Areas (Rural Hospital Obstetric Service Aggregation, Approaches to Risk-Appropriate Care, Financial Sustainability) and provide a detailed plan for addressing the selected focus area(s) within the network.
6. Data collection and sharing: Outline how the network and its members will collect and share aggregate data to document and monitor progress towards the program goals. Explain how this data will be used to assess outcomes and support ongoing support of the network after federal funding ceases.
7. Program Requirements and Expectations: Review and understand the program requirements and expectations, as outlined in the Program Requirements and Expectations document. Ensure compliance with these requirements in the application.
