2. Grant number and title of the specific NIH grant for which the change of recipient organization status is being requested

3. Name of the proposed successor-in-interest organization

4. Contact information of the proposed successor-in-interest organization

5. Reason for the change of recipient organization status

6. Proposed effective date for the transfer of the grant award to the new organization

7. Justification for why the proposed successor-in-interest organization is suitable to continue the project or clinical trial

8. Description of any changes or modifications to the project or clinical trial that are anticipated as a result of the change of recipient organization status

9. Documentation supporting the need for the change of recipient organization status, such as organizational restructuring or acquisition

10. Confirmation of approval from the current and proposed successor-in-interest organizations' authorized officials

11. Statement acknowledging that there is no guarantee of the award being transferred to the new organization

12. Any additional relevant information or attachments that may support the request for change of recipient organization status

CanExport SMEs

Amount of Funding

Up to 50% funding to a maximum $50,000 grant per project;
Applicants may receive up to 50% of eligible costs for up to $10,000 to $50,000 in grant funding per project;
The applicant is responsible for the remaining 50% of costs;
The annual CanExport contribution maximum is $99,999 (in the event of multiple projects approved in the same government fiscal year);
A group of related companies cannot receive >$200,000 in funding in a single fiscal year. Projects budgets should be between $20,000-100,000; and
Applicants can apply for funding multiple times as long as their prior CanExport application has been completed all reporting is finished.

CanExport SMEs

Eligible Applicants

To be eligible for the CanExport program, Canadian businesses must:

Maintain 1 to <500 full-time employees;
Earn annual revenues ranging from $100,000 to $100,000,000;
Be federally incorporated federally or classified as a limited liability partnership (LLP);
Create eligible project expenses of $20,000 or more; and
Be financially stable and able to pay all project expenses not covered by the program.
