4. Planned activities and strategies to address the unmet needs of tribal-specific and culturally appropriate LTSS information and guidance to tribes.

5. Detailed plans for creating a network of navigators through recruitment and training to assist tribes in developing appropriate LTSS.

6. Proposed methods for developing a practical and hands-on toolkit for tribes to implement LTSS based on documented best practices.

7. Description of how the resource center will target its services to the 28 tribes that receive Title VI grants under the OAIANNHP.

8. Plans for publicly available resources that are accessible to all tribes and organizations working with the American Indian, Alaskan Native, and Native Hawaiian populations.

9. Strategies for improving the lives of older adults and people with disabilities through the development of tribal-specific LTSS that are integrated into the unique culture of each tribe.

10. Budget proposal for the grant period, including detailed information on how the funds will be used.

11. Any additional supporting documents or information that may strengthen the application, such as letters of support or previous experience in providing LTSS to tribal communities.

CanExport SMEs

Amount of Funding

Up to 50% funding to a maximum $50,000 grant per project;
Applicants may receive up to 50% of eligible costs for up to $10,000 to $50,000 in grant funding per project;
The applicant is responsible for the remaining 50% of costs;
The annual CanExport contribution maximum is $99,999 (in the event of multiple projects approved in the same government fiscal year);
A group of related companies cannot receive >$200,000 in funding in a single fiscal year. Projects budgets should be between $20,000-100,000; and
Applicants can apply for funding multiple times as long as their prior CanExport application has been completed all reporting is finished.

CanExport SMEs

Eligible Applicants

To be eligible for the CanExport program, Canadian businesses must:

Maintain 1 to <500 full-time employees;
Earn annual revenues ranging from $100,000 to $100,000,000;
Be federally incorporated federally or classified as a limited liability partnership (LLP);
Create eligible project expenses of $20,000 or more; and
Be financially stable and able to pay all project expenses not covered by the program.
